• This offer is available to all clinicians within Portman Dentex that do not currently have an active account with Queensway Dental Laboratory (“Eligible Clinicians”).

• This offer applies to any new Portman Dentex clinician account opened from 1 October 2025 (the “Offer Period”). Queensway Dental Laboratory may, at its sole discretion, elect to extend the Offer Period.

• Eligible Clinicians will receive a rebate equal to 10% of the total invoice value excluding VAT and delivery charges on all spend for the first six (6) months from the account opening date.

• The rebate shall be credited to the Eligible Clinician’s account within thirty (30) days of the six-month period ending.

• This offer shall remain available throughout the Offer Period. Queensway Dental Laboratory reserves the right to withdraw the offer following expiry of the Offer Period, or to extend the Offer Period. • Queensway Dental Laboratory may, at its sole discretion, elect to extend the Offer Period.

• A £250 credit shall be applied to the new account. The credit shall be deducted from each invoice and for clarity, should any invoice be for less than the credit value of £250, the remaining balance will carry forward and be applied against subsequent invoices until the credit value is fully used. Any unused portion of the £250 credit will automatically expire at the end of the Offer Period.

• This offer is available to Portman Dentex clinicians with a live account with Queensway Dental Laboratory, including any new accounts opened between 1 October 2025 (the “Offer Period”). Queensway Dental Laboratory may, at its sole discretion, elect to extend the Offer Period.

• All Posterior Monolithic Zirconia Crowns ordered during this period will be charged at £100 per unit.

• This offer does not extend to orders of Anterior crowns, which will be invoiced at the standard rate.
